New Patient Appointment:
- Fill out paperwork
- Review insurance coverage
- Meet with Doctor to discuss reason for your visit
- Review plan of care/Receive treatment
- Schedule additional appointment(s) if necessary
On average, new patient visits take approx. 30-45 minutes.
What to Bring with You:
- Insurance Card
- Verify that a referral was submitted from your primary care doctor (if your insurance plan
requires a referral). We can assist you to determine if this is necessary for your policy.
- Copay - Due at time of service (We accept cash or check.
If you are unable to remit payment at this time, please notify the receptionist, so that other arrangements can be made).
Personal Injury Patients:
Please notify the receptionist upon arrival if you are: Slip/Fall,
Work Comp, Motor Vehicle Accident Case. We require the following information:
- Date of Accident
- Responsible Party/Insurance Name, Address and Claim #
- Name and Phone Number for Claim Adjuster
- Attorney Name and Phone Number (if applicable)
- Employer Name and Address (Work Comp)
- Any x-ray/MRI reports (if available)
